Photo essay: Female circumcision in Kurdistan

by Amit R. Paley
20-Jan-2009

Originally featured in Washington Post, "For Kurdish Girls, a Painful Ancient Ritual". Photographer Andrea Bruce and special correspondents Nian Ahmed and Dlovan Brwari contributed to this report: Sheelan Anwar Omer, a shy 7-year-old Kurdish girl, bounded into her neighbor's house with an ear-to-ear smile, looking for the party her mother had promised. There was no celebration. Instead, a local woman quickly locked a rusty red door behind Sheelan, who looked bewildered when her mother ordered the girl to remove her underpants. Sheelan began to whimper, then tremble, while the women pushed apart her legs and a midwife raised a stainless-steel razor blade in the air. "I do this in the name of Allah!" she intoned. Kurdistan is the only known part of Iraq --and one of the few places in the world--where female circumcision is widespread. More than 60 percent of women in Kurdish areas of northern Iraq have been circumcised, according to a study conducted this year. In at least one Kurdish territory, 95 percent of women have undergone the practice, which human rights groups call female genital mutilation>>>Full text

Genital mutilation

by Jahanshah Rashidian on

Exact reasons and origin of genital mutilation "circumcision" is unknown. This practice precedes semitic religions. From a cut off the whole penis to a part of it or finally the cut off foreskin, the practice seems originating from sacrificial rituals.

For girls, infibulations “pharaonic” is the most brutal one which means it is at least as old as Pharaohs. In this form, they remove clitoris, labia minora and medial part of the labia majora, with both sides of the female organ stitched together leaving a small opening.

This procedure requires tying together the girl's legs for nearly three weeks. Shocking traumatic and unhygienic treatment often leads to infection and other psychosomatic problems.

Clitoris and foreskin are functional and sensitive organs kept by  evolution and cut off them must be logically and morally banned by appropriate authorities of international community.

Direct coranic verses do not command the practice, but there are a few Hadihs tackling the practice for men and women.

Male circumcision is practised in all Islamic societies, but female circumcision is practised in most of Islamic countries--more than 90% of egyptian women are circumcised.

Circumcision: A Closer Look

by Multiple Personality Disorder on

Copy and paste from Doctor Dean Edell.  Sorry the site software does not allow this text to be pasted with correct setting, if you can read it fine, if not that's fine too.  Although this article was originally published in 1999, he regularly repeats the same sentiment in his radio show up to today, which means all the sientific data presented here could be interpreted differently, which brings me once again to my original question, why not allow children to grow up to adulthood and decide for themselves what they want to do with their own bodies.

{Posting Date: 03/01/1999

I have had a long and vociferous opposition to the practice of routine male circumcision. While many parents in the 60's, in their zeal to return to nature, abandoned the practice, it was 15 years ago that the birth of my fourth son prompted my rethinking of the issue. I had met a nurse, who is now the president of the National Organization of Circumcision Information Resource Centers, who enlightened me to the existing medical data.  The United States was the only country in the world that routinely circumcised all its newborn males. It was a uniquely American ritual. The medical evidence never convinced all the other civilized countries in the world. This did not seem to alter the opinions of medical organizations in the United States. They stubbornly clung to the old myths about circumcision. Well, after analysis of almost 40 years of available medical research on circumcision, the American Academy of Pediatrics has issued new recommendations saying that they do not recommend circumcision as a routine procedure. This is too little and too late for the millions of infants who have undergone this unnecessary and inhumane assault. This is a radical departure for American medicine and, of course, raises many, many issues. With that statement the American Academy of Pediatricians joins major national pediatrics group in England, Australia, Canada, Asia and Europe. COMMON MYTHS Let's first talk about the many myths that one hears. CLEANLINESS The most common myth is that it's cleaner to be circumcised. It's hard to imagine how this has persisted in an era of soap and water. But certainly it's understandable that people do get upset with moist places in the body. A woman's reproductive tract is certainly moist and contains lots of bacteria, yet no one would suggest circumcising females to make them cleaner. CERVICAL CANCER The idea that women who have sex with uncircumcised men will get cervical cancer is a myth that is hard to put to rest. This is so untrue that the Academy did not even consider this in their new evaluation. Cervical cancer is caused by a sexually transmitted virus and has nothing to do with the circumcision status of the male. URINARY TRACT INFECTIONS Some studies have found urinary tract infections to be more common in the intact male baby. Other studies have challenged that.If one accepts the claims of those who think not circumcising increases the risk, then you would have to do 200 circumcisions to prevent one urinary tract infection. COMPLICATIONS The complication rate of the circumcision operation itself - from bleeding to amputation of the penis - is at least one in 200 according to the American Academy of Pediatrics. So you can see that there's no benefit at all.PENILE CANCER When it comes to penile cancer, the Academy notes that it is so rare that it should not be a significant influence on the decision to circumcise. STDs Sexually transmitted diseases (STDs) have been claimed to be more common in the intact male. But is this true? There is no doubt, as the AAP says, that behavioral factors are far more important in determining your risk of STDs and that there are some sexually transmitted diseases that are more common in circumcised men. FATHER TO SON The most common reason men give for circumcising their sons is so they will look like them. This is a poor reason to do this to a newborn baby. DIFFERENCES: SEXUAL SATISFACTION OF WOMEN Recent studies have found that sexually, circumcised men are different from intact men. The glans or tip of the intact penis is more sensitive. Circumcision removes the equivalent of 15 square inches of skin in the adult male. One study recently published in the British Journal of Urology found that intact men are more satisfying to their female lovers than circumcised men. HISTORY Circumcision was initially introduced into this country by reformers who wanted to prevent masturbation, but according to recent studies, if this was the objective, the reformers have failed. PREVENT MASTURBATION? Research published in the Journal of the American Medical Association found that men who are circumcised are more likely to masturbate and to prefer certain sexual practices including oral sex and anal sex.It is possible that the decrease in penile sensation that accompanies circumcision could be playing a role in the seeking of different sexual practices that offer more stimulation. ETHICS Ethically, the AAP walks a fine line. While they admit that parents and physicians have an ethical duty to act in a child's best interest and well being, they fall far short of recommending that we have an ethical duty to leave a child's genitals intact. It seems clear to everybody that female circumcision is a gross and obvious violation of that right, but somehow male circumcision is not seen as extreme an intrusion. The AAP does say that, "parents should not be coerced by medical professionals to make this choice." PAIN The pediatricians finally admit the obvious: that there is considerable evidence that newborns experience extreme pain and significant stress during a circumcision, which unfortunately is performed most of the time without any analgesia whatsoever. Why it's taken so long for pediatricians to admit this obvious fact is a clear condemnation of previous circumcision policy. When I was in medical school, doctors tried to teach me that babies don't feel any pain...  A CHANGING WORLD Internationally about 80 to 85 percent of the male population is uncircumcised. The AAP is not the first, but the fourth major medical society in the world to change its circumcision policy. Australia, the U.K., and Canada already have instituted more humane policies. Hopefully, we will go much further toward stopping this barbaric practice, which is decidedly unhealthy for us and our children.}

first of all, i was specifically comparing infantile circumcision to adult circumcision, not to any other form of surgery, male or female. (on a lighter note, i would love to get into a conversation with a man having either a circumcision or vasectomy to a woman having a c-section. :-0)

ok, that comparison out of the question. with regard to the surgery being more of a risk to an adult male rather than an infant. this is clearly documented.

the below link is a great question and answer:

//www.circumcisioninfo.com/circ_record.html#a...

"Circumcision is a controversial topic because there is so much misinformation and emotion included as fact. The facts are fairly straightforward. There are some definite biological advantages to circumcision. For example, circumcised men don't develop penile cancer- a rare though deadly (and emotionally difficult) cancer. Circumcised men develop fewer sexually transmitted diseases if promiscuous (the skin is thicker and more resiliant to trauma): = quoted reference

Circumcision: A History of the World's Most Controversial Surgery,

"In any case, the evidence is conclusive that circumcision prevents penile cancer just as mastectomy prevents breast cancer. Removing one third to one half of the skin of the penis lowers the odds of contracting what is, after all, a skin cancer. A high percentage of skin cancers eventually develop on the nose, one dermatologist noted; but this has not led physicians to recommend prophylactic rhinoplasties." = quote reference

now, THIS site (below) is almost fanatical in their presentation that circumcision is unnecessary. these guys are almost creepy sounding.

//www.notjustskin.org/en/circumcisionfaq.html

bottom line (again :-): this is a debatable issue. no one is absolutly wrong or absolutely right. it's a personal issue. many men wish they had NOT been circumcised. many other men wish they HAD been at infancy. the entire medical field is not 100% in agreement.

the common yet FIRM answer always comes back to:

circumcision is beneficial but not necessary.

you can draw your own conclusions and decisions.

it is NOT butchering an infant as in female circumcision which is without question butchery.

i can't find the comment now but someone said something about their son's girlfriends not complaining. ewwwwwwww... like i want to hear my son and his girlfriend talking about sex!!!! NOT

The Gate Keepers

by LalehGillani on

Although the male relatives (fathers, husbands, brothers, grandfathers, etc.) of these girls are fully aware of this practice, they are not involved in it. In other words, there is no pressure from the male relatives to circumcised young girls.

The culprits are mainly a Muslim figure head in the local mosque and women themselves. Under the directions of mullahs and their watchful eyes, it is expected that young girls are circumcised quietly, hidden from sight.

One or two women are carefully trained so that the practice may continue over generations. Sanctioned by the local mullah, mothers are brained washed to believe that they are fulfilling a religious covenant and are protecting the honor of the family. At the end, this little girl will grow up to be a good wife and mother and will do the same to her own daughters.

She will become the gate keeper…

Multiple Personality Disorder

Anonymous Fish,

by Multiple Personality Disorder on

Your points are well taken, however I take issue with your none stated conclusion that male circumcision in adults is considered a much greater risk.  As intelligent as you are you make a comparative statement without mentioning what you compare adult circumcision risks to, but I take it that you are implying that adult male circumcision is riskier than infant male circumcision, which I also have issue with, however how about comparing it with a nose job, plastic surgery, Lasik surgery, liposuction, bone surgery, etc.  I had a bone surgery a few years ago where they put me out, open up my skin, muscles, tendons, and nerves to get to the bone, and then they shaved it off, and nothing of ill consideration happened to me.  I believe male adult circumcision is a lot less risky than bone surgery, and I’m sure an adult male would be capable of making that decision for himself.

Also, regarding hygiene of boys, 70% of males in this planet are NOT circumcised, yet they appear to be flourishing, their weewees are not falling off, and when they grow up their wives don’t seem to be whining about preferring circumcised males as the Western women do.  And, on top of billions of male humans there are also billions of other animals like horses, zebra, giraffes, donkeys, and cows that have had foreskins for millions of years and their weewees has not fallen off because something down there might be unhygienic.  And, speaking of hygiene, which one is more unhygienic, an unwashed weewee or a wiped ass with toilet paper?  Why don’t the doctors come up with an idea to clean up boys' asses better if they are so concern about this type of hygiene.

And isn’t it true that most HIV and STD’s are spread by having more that one sexual partner, as most Western woman do, and not by a foreskin.  I’m not advocating a single or multiple partnership, but to say before one (not you personally) blames a piece of skin for spread of any kind of disease and start advocating mutilation of organ parts, they should look at other reasons for spread of diseases.

So back to the original question, why not allow children to grow up to adulthood and decide for themselves what they want to do with their own body parts.

Good reading

by capt_ayhab on

//emedicine.medscape.com/article/1015820-over...

Excerpts:

Suggested benefits and medical indications

Curing
masturbation was historically the most common indication for
circumcision. For many years, both laypeople and medical practitioners
have speculated that circumcision reduces the risk of sexually
transmitted diseases (STDs), particularly ulcerative diseases of the
penis (eg, syphilis). Recent reports in the medical literature support
a protective effect of circumcision (at various levels) against urinary
tract infections (UTIs), some STDs (including HIV infection), cervical
cancer, and penile cancer.

Management of UTIs

UTIs
are more common in male neonates than in their female counterparts.
Studies addressing the association between circumcision status and UTIs
demonstrated an increased rate of UTIs in uncircumcised males,
especially in infants younger than 1 year.

Wiswell and Hockey
(1993) studied 209,399 infants born in US Army hospitals worldwide in
1985-1990. During the first year of life, 1046 infants (0.5%, 550 girls
and 496 boys) were hospitalized for UTIs. Noncircumcised male infants
had a 10-fold increased incidence of infection compared with that of
circumcised male infants.

A meta-analysis of data of 9 studies as
of 1993 revealed a 12-fold increase in the risk of UTIs among
uncircumcised male infants. A study of infants with UTIs showed that
75% of those younger than 3 months were males and, of those, 95% were
uncircumcised. The UTI risk is also increased in premature infants, and
circumcision had a protective effect against recurrent UTIs in this
group of patients.

Although the relative risk of uncircumcised
male infants developing a UTI is estimated to be 4-20 times greater
than in circumcised neonates, the absolute risk of UTIs in
uncircumcised male infants remains low at approximately 1%. Because the
absolute risk is low, recommending routine circumcision in all newborn
males is controversial both medically and ethically. Some children are
at increased risk for UTIs, such as children with neurogenic bladders
who require clean, intermittent catheterization or children with poorly
emptying urinary tracts.

Management of STDs

Mechanisms
proposed to explain the increased risk for STDs in uncircumcised males
include a relatively nonkeratinized inner layer of the prepuce which
increases its susceptibility to minor trauma during intercourse,
allowing pathogens to penetrate through microscopic abrasions. The warm
microclimate created by the preputial pouch permits the microorganisms
to thrive in the smegma that collects in this area.

The most
consistent evidence that supports the association of circumcision with
reduction of the risk of STDs refers to transmission of genital
ulcerative disease and HIV. Eight studies (of variable design) report a
significant 2- to 7-fold increased risk of genital ulcerative disease
(mainly syphilis and chancroid) in uncircumcised male patients (Alanis,
2004).

In the only true meta-analysis to date, Weiss et al
reviewed data from 27 studies and concluded that circumcision
substantially reduces the risk of HIV infection across all populations
evaluated. A study of 187 HIV-discordant couples in Uganda showed no
infections occurring in 50 circumcised HIV-1-negative male subjects
compared with 40 infections among 137 HIV-1-negative uncircumcised male
subjects (Bailey, 1999). Circumcision also decreases the likelihood of
one's acquiring genital ulcerative diseases known to be independent
risk factors for the transmission of HIV.

Management of human papillomavirus and cervical cancer

Human
papilloma virus (HPV) can be oncogenic or nononcogenic. Nononcogenic
HPV (genotypes 6 and 11) cause genital warts in men and women.
Oncogenic HPV (genotypes 16,18, 31, and 33) are responsible for the
great majority of cervical, vulvar, vaginal, anal, and penile cancers.
Circumcision significantly reduces the risk of penile HPV infection in
men and of cervical cancer in the female partners of male individuals
who practice high-risk behaviors such as engaging in sexual activity
with multiple partners.

Management of penile cancer

The
most important factor associated with the development of penile cancer
is an intact foreskin. Wolbarst, who was the first to point out that
Jewish men (the great majority of them circumcised) rarely develop
penile cancer, initially brought this association to the attention of
the scientific community 70 years ago. Recently, a large
population-based study, Schoen et al confirmed the protective effect of
circumcision against penile cancer. Of interest, the other known major
risk factor associated with penile cancer is phimosis, which
circumcision completely eliminates.

poor baby :-(

The most inhumane practice on female organ. Must say that it has nothing to do with religion, it is rather almost prehistoric practice. In fact most religions, including Christianity, Judaism and Islam forbid the practice. The practice has more to do with total barbaric idea of suppression of women through out history than anything else.

As for male circumcision, according my son who is a practicing surgeon in Boston Medical, Jury is still out about the benefit/harms of male circumcision. One thing is known for fact, once it is preformed by a professional, there is no pain nor any damage to the organ.

As Anonymous fishie pointed out, in fact majority of women prefer circumcised male sexual partner. Both statically, and hygienically ;-)

erect vs soft

by Alborzi (not verified) on

You are bringing good points, but mostly based on common misunderstanding. In fact most European/South American males are uncircumcised, yet there is no higher infection rates reported. This is not a scientific research, but the foreskin may protect the penis as much as gathering germs. So the net effect may be negligible.
About the looks of it, you are basing your analysis based on its look when it soft, in fact after erection its hard to tell them apart, also your feeling might be based on familiarity, most girl friends have not complained about my sons.

too much mixing of of separate issues

by Anonymous fish (not verified) on

ok... i think we all agree that circumcism in young girls is barbaric. It's not practiced in Iran. Period. End of story. Anyone who would do something like this or approve of it being done should be punished.

As for male circumcision. Statistics change every single day. Reports from 2003 as orignally posted are constantly updated. I'm waiting to hear back from two medical sources just to get their feedback (male doctor with 3 boys and female nurse/administrator with 2 boys). Concensus is mixed. Nothing in the world is absolute, except death and taxes. :-)

Originally circumcision was done for several reasons, hygiene AND for medical safety reasons. Whoever pointed out that HIV is only 50 years old is correct, but penile cancer and simply infections are ABSOLUTELY A RESULT OF CARELESS HYGIENE. And even with the knowledge that personal hygiene could reduce the possibility of infections, it is an undisputable fact that MOST young boys do NOT take care of their hygience sufficiently. I don't know why anyone would argue this. You can't run around your son after he's 8 years old making sure he's cleaned himself correctly. If you say you can, you don't have an 8 year old boy! At this point it becomes more of a convenience, unfortunate as that may be.

Secondly, appearance. I'm not going to speak for middle eastern women so don't go jumping my shit. but i will tell you that the majority of western women prefer a circumcized male. sorry, it's the truth. (i just hope some man comes in and starts blasting away about THAT so I can point out some of THEIR expectations are hardly in the best interest of woman. :-0)

Also. you can cherry pick from zillions of article on the internet. some say it's harmless and minimally effective, some say it's painful and has zero benefit, and some still stand their ground that it's necessary.

It was quoted that circumcision is beneficial, not necessary. I think that's about as specific an asnswer as you're going to get. NO respectable physician will say it's absolutely unnecessary. There are extremists in every walk of life, even the medical profession. You can always find doctor some where that will tell you exactly what you want to hear.

And I repeat that circumcision in adult makes is considered a MUCH greater risk.

There was this guy who was convinced he got the pain in his kidney because he had to use the old fashioned toilet, in fact he had a kidney stone. Every day millions have it and it has nothing to do with toilet. To jump to a conclusion based on some trends is not scientifically correct. For example most males are circumcised in the USA, but its not even a Christian requirement. Its not an Islamic requirement to do it, now Somalis do it for various reason and they happen to be Moslems too, but that is a coincidence. Now if it helps your arguments, go listen to a religious fanatic they also see coincides as scientific, you guys might like each other, as they say do not knock it before you try it.
